Add serviceaccount support to common and appmgr

This change adds a serviceaccount to the appmgr helm chart with explicit
permission to list pods in the namespace where it expects to find its
tiller; this is to handle the case where the default serviceaccount is
not allowed to list pods.

To support his, service account name support was added to the common
chart.

Credentials are per namespace therefore we need to deploy this chart
for each namespace where the credential is needed.  The helm release
name used for deploying the chart to each namespace should reflect the
namespace; otherwise we will have "release already exists" error when
deploy the credential into the second namespace in the same cluster.

Previous method fo rgetting floating IP address is OpenStack specific,
not working for virtualization platforms such as VirtualBox.  Switching
to a more generic method.

FEATURE: General-purpose RIC platform initContainer image

This change adds a docker image intended for use as an initContainer
for RIC platform components.  The container is intended to have only
those components required for preconfiguration of RIC platform
element pods and two shell scripts:

/ricplt-init.sh is the script which should implement pod initialization

It is assumed that individual RIC component charts will overlay
/ricplt-init.sh with a component-specific initialization script.

This patch introduces a new structure of the it/dep repo.

Components are restructured into a tree. Helm charts are
designed with common templates so that parameters can be
standardized.

This patches introduced installation scripts that take override
value.yaml files for all the helm charts.
